MATERNAL CHILD NURSE EDUCATOR

OVERVIEW

The Great Expectations Nurse-Family Partnership (NFP) program provides case management and education through home visitation for low-income, first-time mothers and their families between Aspen and Parachute, Colorado. Nurse visitors conduct in-home and virtual visits from the early prenatal period to the child’s second birthday. QUALIFICATIONS AND SKILLS

Applicants must be registered nurses; have a current Colorado nursing license and CPR certification; have knowledge of nursing practices; and have experience working with pregnant clients, infants, and toddlers. To perform this job, nurse visitors must have a valid Colorado driver’s license; use of a personal vehicle; and current auto insurance, as required by state law. Bilingual (English/Spanish) ability preferred and compensated accordingly.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Establish and maintain therapeutic relationships while delivering individualized client case management services.
• Conduct home visits to determine client and family needs. Assess the physical, emotional, social, and environmental needs of clients and their families as they relate to health and life-course development and develop plans to meet these needs according to program guidelines for a caseload of 25+ clients. Evaluate progress of family members toward targeted goals and revise plan of care, as appropriate. 
• Provide support to assist clients and their families in attaining targeted goals in areas that include prenatal care, nutrition, parenting, well-child care, family planning, and special healthcare needs.
• Teach clients about healthy behaviors, provide technical expertise to clients regarding best practices for appropriate provision and care for their child’s/children’s and family’s health and well-being. Assist clients and their families in establishing realistic goals and measurable outcomes for promoting their health and life-course development.
• Develop an effective working relationship with clients and their families to enhance problem-solving competence. Use logic and reasoning skills to identify the strengths and weaknesses of each family and guide them to consider alternative solutions or approaches to problems when appropriate. Consider the relative costs and benefits of potential actions to help the family choose the most appropriate solution for them. Promote the involvement of other client family members during pregnancy, birth, and early childhood.
• Provide education, resources, and referrals as appropriate. Identify gaps and barriers in community systems for clients and families. Consult, coordinate and collaborate closely with community resources to link families with appropriate services. Refer clients with social and emotional problems to other community agencies for assistance. Serve as a resource to other community agencies.
• Responsible for the documentation for assigned clients and maintain records of patient care, condition, progress, or problems to report and discuss with supervisor.
• Work independently while maintaining strong professional boundaries.
• Maintain current nursing license.
• Maintain sensitive and confidential records information.
• Drive for extended distances and periods of time from Aspen to Parachute in all weather conditions throughout the year.
• Comply with federal, state, and local regulations and employer policies and procedures.
• Perform other duties as required.

JOB TYPES: full-time (Monday through Saturday, 40 hours/week)
